Registration Process
As a way to streamline the collection of necessary information, BIA has moved to an online registration process that all parents/guardians will need to complete.
*ALL PAPERWORK & FORMS ARE DUE WEDNESDAY, JULY 15TH!*

New Families Only
Must submit an Original Birth Certificate
Every student that is new to B.I.A. must submit an original Birth Certificate during the registration period. We will make a copy for our files and give the original back.*We cannot accept Birth Certificates electronically*
New & Returning Families

ALL FAMILIES:
This year our Registration Forms are available online! We will not send out paper copies of any forms. They must all be received electronically.

IL Health Exam Form: All Students at Bethesda International Academy are required to have a recent health exam AND up-to-date immunizations on file every year . Health Exam Forms are valid for 13 months. Please have your child's Pediatrician complete this form at their annual check up.

Concussion Protocol Form:
KINDERGARTEN THROUGH 8TH GRADE STUDENTS

Eye Exam Form: Required for students entering: KINDERGARTEN

Tuesday, August 18th from 5:30 - 7:00pm
Supply Drop Off, Meet the Teachers & Curricular Fee Payment Due
Wednesday, August 19th from 8:45am-12:00pm
FIRST DAY OF SCHOOL!!! – We will be on a half day schedule.
*After school care is not available this day.*
Friday, August 21st from 5pm-7:30pm
